23-24 CBA Permission Slip – Transportation from HABC to FBC for Karate/Football

Dear parents,
The afterschool activity for which you have registered your child will meet at the FBC campus (5032 Lackawanna Blvd, North Charleston, SC 29405). CBA is able to provide transportation from the HABC campus (4602 Durant Ave, North Charleston, SC 29405) to the FBC campus on the days that their class meets.

Students will be driven to the address above on the CBA bus OR in the vehicle of a CBA teacher or staff member. The school’s liability insurance does not cover personal vehicles. The car insurance of each driver covers the passengers in the vehicle.

By signing this form, you acknowledge:

- understanding of the benefits and risks associated with this trip;
- that you have carefully read and signed the FIELD TRIP LIABILITY CONSENT AND RELEASE FORM for the 23-24 school year and agree to its terms knowingly and voluntarily;
- that your child has permission to ride on the CBA bus OR in a personal vehicle driven by a CBA teacher or staff member; and
- that you are the parent/guardian of the student named on this form.
